All patients must complete our COVID-19 Screening Questionnaire

Due to COVID-19 restrictions, we ask that you only bring ONE visitor with you in the facility

Please note that masks ARE required inside the facility.  You will need to bring a mask for yourself and your visitor

If you have been vaccinated against COVID-19, please email a copy of your vaccination card to nurses@pscaustin.com

If you ARE NOT vaccinated against COVID-19, you are required to have a COVID-19 test (PCR or antigen) within 72 hours of surgery.  This test needs to be administered in a facility by a medical professional. You will need to email nurses@pscaustin.com with a negative result in order to proceed with surgery. Please note: We CANNOT accept any type of at-home test.

Please be sure to quarantine after taking your test until you come in for surgery
